An adult must be present during the appointment .
Phlebotomy collection at patient’s requested location with delivery to local laboratory. All collections require signed order from medical physician, nurse practitioner, or physician assistant. "Please note: An After Hours Fee will be applied to any appointments scheduled or completed outside of our standard operating hours."

Cancellations: Fees may be partially refundable if a cancellation occurs before the collection kit is dispatched or the appointment is attended.
Non-Refundable Items: Once a laboratory analysis has begun or a sample has been collected by a third party, fees are generally non-refundable due to the administrative costs of the legal chain of custody.
